What to Submit with an application for Passport
- Each applicant must be present
- Original proof of U.S. Citizenship (Long form Certified birth certificate or certificate of Naturalization)
- Copy of proof of U.S. Citizenship (Long form Certified birth certificate or certificate of Naturalization)
- Proof of identity (Texas Driver License, or previously issued U.S. Passport Book/Card)�with a clear copy of the front and back
Any proof of identity other than a Texas Driver License, or previously issued U.S. Passport Book/Card will require additional documentation. - One recent color photo (Passport Size) 2×2-No Eye Glasses
- Two checks or Money orders
- Completed application.
(No Cash, No Credit Cards or Temporary Checks Accepted)
